I had been on FMLA for years.  Before it was for migraines and the last few years it was because of my back and fibromyalgia.  It has to be renewed every year where I work.  I could call off or go to a doctor and the time off was always given to me.  The 12 week period is the length of time before the fmla benefit wears out.  I went over that 12 week limit last year and almost lost my job.  I had to go on a medical disability or they called it an early retirement.  This way my employer has to hold my job for me up to 5 years should I be able to return to work.  After that time, I am considered fully disabled until I am 63 and then I am eligible for my retirement.

The fmla helped me during times I needed off work and didn't have any sick or vacation time accumulated.  My job did not have a leave witout pay clause.  I did not get paid when I was off work without any sick/vac time accured.

Finding a diagnosis has helped in the treatment of my many ailments and the placquinil has helped me alot.  Overall I would say that I am 70% better than I was last year.  But, keep in mind, I was forcing my body to work and take care of my family too.  I see my rheumy in June and she wants to see if the improvement is still the same.  If so, I may go back to work and there are ADA laws to help protect me then.  (ADA - American Disibility Act)

Hope this helps......
