Can we create a new pain scale?

Currently the Dr use a 1-10 scale with smilies.
10 - Suicide
8 - Black out
6 - destroy anyone who touches you
4 - don't talk to me

I think that you get the idea.

What are your suggestions?

7=8,, Have security remove my hands from your throat
9-10,, Security didnt make it in time
