I have had a position ANA titer of 1:2560, 1:1280, and 1:640 over the past five years. It never goes lower than 640. I just had my SSA/SSB tested again and they are both still negative? This is so frustrating. Do I have SS or not? My Dr. said I have hallmark SS symptoms and I have a low C3 compliment, and he thinks it is SS.
Anyone else, ever deal with this? He said in time we will do a lip biopsy.

Only 70% patients have ssa positive.
In sjogren patients with neurological involvement only 20% have ssa positive at onset...... After 10 years only 40% ssa positive.
If you list down your symptoms i might share what i know from my research.
Also tell me if you have done the rps inflammadry test done and its result.
This was similar to my case, except my C4 was at bottom range. My IgG1 was also above range. These abnormalities follow what's known to be a typical profile for autoimmune... the lip biopsy is the final piece used to uniquley determine Sjogrens as the differential diagnosis from the other diseases. (Besides your obvious symptoms).
I'd say do it, my biopsy was done at JH... I recovered nicely and it only bothered me for two weeks. Good luck!

My symptoms:
tingling and numbness in the legs (comes and goes)
Trouble swallowing (comes and goes)
Migraines
Dry eyes, dry mouth
Geographic tongue
dizziness (comes and goes) Like a funhouse, like the floor is one step lower than I think. It is weird
Extreme fatigue (comes and goes)
Muscle pain/joint pain
Stomach pain
I have had 3 MRIS, endoscopy, barium swallow, you name it. Haven't had the rps inflammadry test. My mom and brother both have RA and my aunt has Lupus.
