The CVS Caremark Charitable Trust has announced grants totaling more than $1 million to community health centers nationwide to increase access to quality health care and improve health outcomes while reducing costs.
Awarded as part of a $3 million, multiyear initiative in partnership with the National Association of Community Health Centers, the twenty-one Innovations in Community Health grants will support the development of community-based programs and initiatives focused on the treatment and management of chronic illnesses such as heart disease, diabetes, hypertension, and asthma.
